Physical Examination Requirements

All student-athletes must complete and submit all required forms through the Athletic Physical Portal (Bulldog Doctor) before they are permitted to participate in tryouts, practices, or competitions.

• Physical examinations must be completed and submitted for review and approval.
• No student-athlete will be cleared until all required documents are submitted and approved.
• Incomplete or missing forms will delay athletic clearance.

Validity:

• Sports physicals are valid for one (1) year from the date of the examination.

Important Clearance Notice

ALL STUDENT-ATHLETES MUST HAVE ALL REQUIRED DOCUMENTS COMPLETED AND SUBMITTED IN THE PHYSICAL PORTAL IN ORDER TO BE CLEARED FOR PARTICIPATION.

Failure to complete every required form in the portal will result in delayed or denied clearance, even if the student has been medically examined.

Alternative School Students

Student-athletes who attend an alternative school must:

• Complete the NJSIAA Revised Physical Packet
• Submit all completed paperwork directly to the Ferris High School Nurses for review

Submission to School Nurses

All high school pre-participation physical exams and health history questionnaires must be returned to the school nurse in the appropriate building:

• Freshmen & Sophomores: Main Building
• Juniors & Seniors: JA Building
